All the primary duties within this document will be performed according to established policies, procedures and guidelines within the department and the Medical Center. JOB SUMMARY: The RN Case Manager is a professional RN with strong clinical knowledge and leadership skills who works under the supervision of the Director. Collaborates with the patient, family, physician and interdisciplinary team and payer to plan, organize, coordinate and monitor the provision of cost-effective care, ensuring quality health care services and resources needed to promote optimal outcomes (clinical, functional, financial satisfaction) PRIMARY JOB DUTIES: 1. Assesses the health status of patients assigned to the CHF Program including data from history, physical, assessments, interview with patient/family to determine care needs related to the medical diagnosis, treatment options, financial resources, educational needs, psychosocial needs and discharge planning. 2. Makes initial and follow up phone calls to patient/family enrolled in the program to assess their level of health literacy and to establish and implement the case management plan of care. 3. Assists patient/family in decision making and priority setting to ensure compliance with evidence based treatment plan and optimal quality outcomes. 4. Coordinates care for enrolled patients to assure optimal outcomes, including decreased readmissions and LOS, cost containment, and customer satisfaction. 5. Educates the patient/family, and care providers regarding the care expectations of the patient population. 7. Identifies and tracks readmissions, evaluating the reason for readmission and provides potential preventative solutions in collaboration with physicians, patient care staff, case managers, nursing leadership and outcomes managers. 8. Assists in the development and implementation of patient pathways and care standards specific to the needs of the CHF patient population. Routinely evaluates patient pathways and in collaboration with the multidisciplinary team revises pathways based on qualitative and quantitative outcomes data. 9. Actively contributes to the review/revision/implementation of polices and procedures that relate to the CHF patient population. 10. Participates in performance improvement activities based on the CHF patient population by identifying barriers to care, systems and process issues, and contributing to data collection and analysis for improvement plans. 11. Develops collaborative relationships with other departments, entities, and external health care agencies to facilitate and support quality of care. 12. Demonstrates effective communication skills to collaborate with a variety of persons, including the patient, public, coworkers, guests, payers, outside agencies and medical staff. 13. Acts as a clinical/educational resource for patients, families, interdisciplinary team members and payers. 14, Organizes and performs work responsibilities effectively and efficiently. 15. Demonstrates the standards of performance (ownership, teamwork, communication, compassion) that support patient satisfaction and the principles of service excellence. 16. Performs other related duties as required. ESSENTIAL JOB SPECIFICATIONS: 1. Education: Graduation from accredited School of Nursing; BSN or Bachelor's degree in a related field or 8 years experience as an RN Required. 2. Licensure / Certifications: Current license to practice professional nursing in State of North Carolina. Certification in Case Management, or clinical specialty preferred. 3. Experience: Minimum of 5 years experience as an RN required. Must be able to demonstrate expertise in a clinical specialty for defined population.
